Food Safety Week

Now that the weather is improving, lots of us are taking the opportunity to have barbecues and eat outside.   Please be careful to avoid getting ill with food poisoning.  The main dangers are not cooking meat properly and cross-contamination between cooked and raw food.   You can find lots of advice including recipes to help prevent this type of illness at http://www.food.gov.uk/safereating /hyg/germwatch/2010

You can find our registration forms below. Click on a link to open a form. You can then print it, fill out your details and bring it along to the practice with your NHS Medical Card to arrange an initial consultation. Please note Patients are registered with the practice, not an individual GP.

Your medical card will be issued in the name of one of the doctors who will supervise your care, however you can at any time express a preference for a practitioner, for either all of your medical needs or on a case by case basis. This preference will be recorded in your medical notes. Whilst the practice will try to comply with your requests on all occasions, it may not always be possible 

Anyone living within the practice area may register with this practice.   When registering, it is helpful if you can bring your medical card and birth certificate with you (as well as those of any other family members that you are registering).  

In addition to the registration form, you will be asked to complete a new patient questionnaire.  This is just to provide some information about yourself which will  help the doctor to provide medical care for you while your medical records are being transferred from your old practice. 

If you move out of the practice area, you will need to register with a new practice as soon as you move.   If you need help finding a new practice, contact the local NHS Health Board
